Support Coordination Services,
SSS will be there to be a strong advocate for the people they serve.
Private support coordination
in Utah
will allow for more stability by allowing individuals and families to choose and keep their Support Coordinator
or change Support Coordinators if they wish.
There is no cost to the individual
and family or decrease in services by choosing a private support coordinator, who are contracted to coordinate the delivery
of supports to persons receiving services, monitor the health and safety of those persons and report to Utah DHS/DSPD.
All companies who offer Support
Coordination services have agreed to follow procedure, in thier contract with the State of Utah to allow individuals to chose
and or change Support Coordinators anytime;
Here are a few ways to select
a Private Support Coordinator
1st) The Individual or
Family can select a SCE's from a DSPD list, searching the internet, asking for recommendations from people
you know, or other source you have researched.

Contact a potential New Support Coordinator(SCE) and schedule an interview.

After Interviewing Support Coordinators make a selection and verify that the New Private
Support Coordinator or company can still take the new individual (each SCE is allowed up to 46 cases).

After choosing a New SCE the individual or family should call the Utah DHS/DSPD
Supervisor to have the case transferred.

With DHS/DSPD authorization, cases are transferred to the new SCE. (try to give
DSPD 10-15 days to get approvals)
or 2nd) Call the DSPD Supervisor and let them know you want to make a change by sending
out an Invitation for Service Offer (ISO).

Interview Support Coordinators that responded to the ISO, make
a selection and verify that the New Private Support Coordinator or company can still take the new individual (each SCE is
allowed up to 46 cases).

With DHS/DSPD authorization,
cases are transferred to the new SCE.
